Job Description:

We are seeking a compassionate, organized, and dedicated Social Worker to join our Adult Day Health Care Center team. The Social Worker will support our participants by addressing their social, emotional, and practical needs while working closely with families, caregivers, and the multidisciplinary care team.

The ideal candidate will have experience working with older adults and/or individuals with disabilities, strong communication skills, and a genuine desire to improve the quality of life of our participants.

Hours 8am-2pm

Job Requirements:

* Bachelor’s or Master’s degree in Social Work from an accredited program.
* Experience working with older adults, individuals with disabilities, or in a healthcare/community-based setting preferred.
* Strong communication, organization, documentation, and interpersonal skills.
* Ability to work effectively as part of a multidisciplinary team.
* Compassionate, patient, and respectful approach to working with elderly and vulnerable populations.
* Knowledge of community resources and social service programs preferred.
* Bilingual/multilingual skills. Must speak English and Armenian.
