Job Description:

We are seeking a compassionate and highly organized Social Worker to join our Adult Day Health Care and Enhanced Care Management (ECM) team in North Hollywood.

The Social Worker will support ADHC and ECM participants and their families by conducting assessments, developing individualized care plans, completing daily flowsheets and progress notes, providing case management, coordinating community resources, and offering emotional support and counseling as needed.

Schedule:

Monday–Friday, 8:00 AM – 3:00 PM (in person)

We are looking for someone who works well under pressure, is team-oriented, and is passionate about serving vulnerable populations.

Job Requirements:

Master’s degree in Social Work, Psychology, Sociology, Counseling, or related field

Bachelor’s degree in Social Work

Qualifications:

• Strong organizational and time management skills

• Ability to work efficiently in a fast-paced environment

• Quick learner with strong computer skills

• Valid driver’s license

• CPR and First Aid certified

• Excellent English verbal and written communication skills

• Fluent Armenian required; Russian and/or Spanish is a plus

Salary:

$25–$30 per hour (depending on degree), + ECM compensation Earn an additional $20 per hour
