-Provide ongoing support, counseling, and crisis intervention to participants and their families or caregivers.

-Assist participants and families in accessing community resources such as transportation, home care, housing, financial assistance, and mental health services.
-Advocate for participants’ rights and well-being in accordance with ethical and legal standards.
-Facilitate support groups, therapeutic activities, and educational sessions for participants and caregivers.
-Participate in interdisciplinary team meetings to review and update care plans.
-Maintain accurate documentation of assessments, interventions, referrals, and case notes in compliance with organizational and regulatory guidelines.
