Job Description:

We are looking for a reliable and organized Hospice Intake coordinator.
This position is responsible for scheduling patient visits, communicating with doctors, hospitals, and Hospice staff.

* Schedule patient visits and assign clinicians
* Communicate with physicians, hospital discharge planners, and staff
* Enter and update patient information in DSL and Synergy systems
* Ensure all required documents are received and complete
* Follow up on referrals and authorizations
* Maintain accurate records and timelines

Job Requirements:

* Fluent in English
* Strong communication and organizational skills
*Experience in office work preferrable
