Position 1: Family Care Nurse Practitioner (FNP)
Position Summary

Provide comprehensive primary care services, including assessment, diagnosis, treatment, prescribing, education, and care coordination for patients across the lifespan.

Minimum Qualifications

• Active and unrestricted California Nurse Practitioner license
• National NP certification as an FNP (AANP or ANCC)
• DEA registration (or eligible to obtain)
• Current BLS certification (ACLS preferred if applicable to clinic scope)
• Strong English communication skills and strong written English for medical documentation

Key Responsibilities

• Perform patient evaluations and develop treatment plans
• Diagnose and manage acute and chronic conditions within scope
• Order and interpret labs and diagnostics, follow up on results
• Prescribe medications and therapies as appropriate
• Provide preventive care, screenings, and health education
• Coordinate referrals and collaborate with clinical team
• Complete accurate, timely documentation in the medical record system
• Support quality, safety, and clinic workflow improvements

Preferred

• Community clinic or underserved care experience
• Spanish or Armenian language skills
• EHR experience

Position 2: Medical Assistant (MA)
Position Summary

Support providers and patients by completing clinical tasks, assisting with patient flow, and helping maintain efficient, safe clinic operations.

Minimum Qualifications

• Completion of a Medical Assistant program (certificate or diploma)
• MA certification preferred
• Current BLS and CPR certification and Phlebotomy
• Strong English communication skills and strong written English for charting and instructions

Key Responsibilities

• Room patients and document vitals, medications, allergies, and visit details per protocol
• Assist providers during exams and procedures within MA scope and training
• Perform tasks as trained and competency verified, which may include injections, EKG, specimen collection, point of care tests, and phlebotomy, depending on clinic needs
• Keep exam rooms clean, stocked, and prepared
• Support documentation and EHR workflows as assigned
• Provide patient instructions and assist with follow ups, referrals, and coordination tasks
• Follow infection control and safety protocols

• Community clinic experience
• Spanish or Armenian language skills
• EHR and strong typing skills
