Job Description:

Our Home Health agency is growing and we’re looking for an experienced LVN who wants
to move from the field into a stable office position. You’ll help support our intake team and
make sure new patients are set up correctly and quickly. This is an in-office role that plays a
big part in keeping the agency running smoothly.

What You’ll Do:
• Review new referrals and make sure they’re appropriate for Home Health
• Help organize admissions and start of care
• Communicate with doctors’ offices for orders and signatures
• Support chart accuracy and clinical documentation
• Help with intake calls when needed
• Work closely with nurses and office staff to keep admissions moving
You support the clinical process, but RN leadership remains in charge.

Job Requirements:

• Active California LVN license
• At least 1 year of Home Health or Hospice field experience (required)
• Comfortable with Medicare Home Health basics
• Strong English communication skills
• Organized and dependable
• EMR experience is a plus

Schedule:
• Full-time
• Monday–Friday
• In-office

Compensation:
• $28 – $35 per hour, depending on experience
• Room to grow as the agency expands
