Hello, we are seeking a dedicated LVN Care Coordinator to join our growing home health team. This position works under supervision and plays a key role in coordinating patient care services to ensure high-quality, timely, and efficient care delivery.

Job Requirements:

The candidate must have prior home health experience, strong computer and communication skills, and the ability to multitask in a fast-paced environment. The LVN Care Coordinator will assist with care coordination, documentation, and communication with clinicians, patients, physicians, and hospitals, ensuring timely follow-up on all patient needs.
