Key Responsibilities:
Coordinate patient admissions, Start of Care (SOC), Resumption of Care (ROC), Recertifications, Transfers, and Discharges
Track and coordinate OASIS documentation and ensure timely completion by clinical staff
Monitor Medicare documentation requirements and assist with maintaining compliant patient records
Follow up on Plans of Care, physician orders, verbal orders, and outstanding signatures
Coordinate nursing, therapy, and other home health visits according to the patient's Plan of Care
Review patient charts for missing, incomplete, or outstanding documentation
Communicate with physicians' offices regarding orders, signatures, and patient care needs
Coordinate new referrals and assist with the intake and admission process
Maintain communication with patients, families, nurses, therapists, physicians, and other healthcare providers
Track hospitalization, transfer, discharge, and changes in patient status
Assist with ensuring documentation is completed within required Medicare and agency timeframes
Support compliance with Medicare Conditions of Participation and applicable home health requirements
Maintain accurate and confidential patient information in accordance with HIPAA
Work closely with clinical and administrative staff to support continuity and quality of patient care
Qualifications:
Previous experience in home health coordination is strongly preferred. Candidates should have familiarity with OASIS, Medicare home health documentation, Plans of Care, physician orders, and home health clinical workflows.
The ideal candidate is highly organized, detail-oriented, dependable, and able to manage multiple patients and deadlines while maintaining professional communication with clinical staff, physicians, patients, and families.
