INTAKE/COORDINATOR NEEDED FOR A HOSPICE AGENCY ASAP! - LOS ANGELES,CA

Job Description:

Coordinate hospice services for patients with terminal illnesses.

Ensure patients receive appropriate pain management and symptom control.

Develop and implement individualized care plans in collaboration with the hospice team.

Monitor patient condition and communicate updates to the interdisciplinary team.

Ensure timely admissions, recertifications, and discharges.
Work closely with:

Physicians

Nurse Practitioners

Registered Nurses

Social Workers

Chaplains

Hospice Aides

Participate in Interdisciplinary Team (IDT) meetings.

Ensure all disciplines follow the patient’s care plan.

Job Requirements:

Must have prior experience working with a HospiceMD and understanding of hospice physician orders, certifications, and care planning.

Demonstrated ability to manage and coordinate hospice patient care effectively, including admissions, recertifications, interdisciplinary team communication, and scheduling of services.

Strong organizational and leadership skills with the ability to coordinate between nurses, physicians, social workers, and hospice aides to ensure timely and appropriate patient care.

Knowledge of hospice regulations, Medicare guidelines, and documentation requirements is highly preferred.

RN (Registered Nurse) or LVN (Licensed Vocational Nurse) license is a plus, but not mandatory depending on experience.

Excellent communication skills when working with patients, families, and the hospice care team.

Ability to work in a fast-paced hospice environment while maintaining accurate documentation and compliance.
