Intake Coordinator Needed for Home Healthcare Agency - Van Nuys, CA

Responsible for managing the patient intake process from referral receipt through admission, ensuring timely coordination of care, insurance verification, physician communication, and regulatory compliance. Serves as the primary liaison between referral sources, physicians, patients, caregivers, and clinical staff to facilitate seamless admissions and ongoing care coordination.

We're looking for a self-starter who takes ownership and treats the business like their own — someone who moves fast, solves problems independently, and doesn't need to be micromanaged.

Essential Duties & Responsibilities:

- Receive, review, and process incoming patient referrals from hospitals, physicians, clinics, case managers, and other healthcare providers
- Verify Medicare eligibility and benefits
- Confirm physician orders, Face-to-Face documentation, and required admission paperwork are complete and compliant
- Coordinate Start of Care (SOC), Resumption of Care (ROC), Recertification, and discharge processes
- Communicate with physicians, referral sources, patients, caregivers, and field clinicians regarding admission status and care coordination
- Maintain accurate patient records within EMR systems while ensuring HIPAA compliance
- Assist with OASIS-related documentation tracking and follow-up
- Coordinate services including Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, and Home Health Aide services
- Collaborate with agency leadership to ensure timely admissions and optimal census growth
- Follow up on pending referrals, missing documentation, and physician signatures
- Support accreditation, licensing, and Medicare survey readiness by maintaining accurate records and compliance standards
