This full-time role is Monday–Friday, 9:00 AM to 5:00 PM (non-negotiable) - Manage patient intake and referrals efficiently using Synergy - Oversee office scheduling and staff coordination - Maintain accurate records and documentation - Support smooth daily operations of the agency |

Job Requirements: Minimum Requirements: -Minimum 2 years’ experience using Synergy software -Minimum 1 year of experience in Home Health as an intake coordinator -Strong knowledge of intake coordination, patient admissions, and office scheduling -Excellent organizational and communication skills -Ability to manage office operations and support compliance requirements
