We are seeking a compassionate, organized, and motivated Hospice Case Manager / Intake Coordinator to join our team. This role combines clinical case management with intake coordination to ensure a smooth transition for patients and families entering hospice care. The ideal candidate is a skilled communicator, empathetic caregiver, and strong advocate for quality end-of-life care.
Manage the intake process for new hospice patients, including gathering referral information, verifying eligibility, and coordinating admissions.

Serve as a primary point of contact for patients, families, physicians, and referral sources during the admission process.
English is a must, and Spanish is preferable 

Experience in a hospice intake and   case management is a must

Conduct initial assessments and develop individualized plans of care in collaboration with the interdisciplinary team.

Provide case management for an assigned caseload of hospice patients, ensuring comprehensive, coordinated care that meets patient goals and hospice standards.

Maintain timely and accurate documentation in accordance with organizational policies and regulatory guidelines.
Participate in interdisciplinary team meetings, providing updates and collaborating on patient care plans.
Educate patients and families on hospice philosophy, services, and what to expect during hospice care.
Support quality assurance and performance improvement initiatives as needed.
