Hospice Nurse Practitioner Needed

Job Description:

We are currently hiring for a Hospice Nurse Practitioner. Nurses will visit patients (in-home or facility) and work as a team to collaborate by providing exceptional care to our hospice patients and healthcare professionals.

The Hospice Nurse Practitioner regularly reviews and updates the plan of care, performs prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations, and provides education and supportive care to patients and caregivers.
Will communicate effectively with patients and provide daily and weekly updates to the Doctor who oversees the Hospice Care plan.

Job Requirements:

California NP License
-Basic Life Support (BLS) certificate required; additional certifications may be preferred.
-Current nursing License in the state of CA.
-Proficient in using medical equipment and technology relevant to the position.
-2+ years of relevant work experience (hospice or equivalent)
