Receive, review, and process home health referrals from hospitals, physicians, facilities, health plans, and other referral sources.

Enter and maintain accurate patient and referral information in Data Soft Logic.

Verify patient demographics, insurance information, eligibility, benefits, and authorization requirements.

Review referral documentation to ensure required information is complete.

Coordinate with clinical staff regarding patient eligibility and appropriateness for home health services.

Communicate with referral sources regarding missing documentation, orders, authorizations, and admission status.

Coordinate start-of-care scheduling with clinical staff and patients.

Track pending referrals and follow up promptly to avoid delays in admission.

Maintain accurate referral and admission records.

Monitor the intake pipeline and provide updates regarding pending, accepted, and declined referrals.

Assist with obtaining physician orders and required clinical documentation.

Coordinate with billing, clinical, and administrative departments as needed.

Maintain patient confidentiality and comply with HIPAA requirements.

Perform other intake and administrative duties as assigned.

Qualifications:

Fluent in English

Previous home health intake experience required.

Hands-on DSL experience required.

Knowledge of the home health referral and admission process.

Understanding of Medicare, Medi-Cal, managed care, and/or commercial insurance requirements.

Experience verifying insurance eligibility and benefits.

Strong computer and data-entry skills.

Excellent attention to detail and organizational skills.

Strong written and verbal communication skills.

Ability to manage multiple referrals and deadlines in a fast-paced environment.

Ability to maintain patient confidentiality and handle sensitive information appropriately.

Preferred Qualifications

Experience with Medicare-certified home health agencies.

Experience with managed care authorizations and LOAs.

Experience working with hospitals, physician offices, SNFs, and other referral sources.

Familiarity with OASIS and home health documentation requirements.

Experience coordinating admissions and start-of-care visits.
