We are seeking a motivated and detail-oriented home health intake coordinator with at least 1 year of experience in home health intake position.

Responsibilities:

Manage and process patient referrals and intake coordination
Coordinate with healthcare providers, patients and agencies
Ensure timely and accurate data entry and follow up

Job Requirements:

Requirements:

Minimum 1 year of intake experience in home health
Fluent in English (spoken and written)
Experience with Synergy Health Care is preferred
Strong communication and organization skills
Reliable and able to work in a fast-paced environment
