Job Description:

We are looking for a reliable and organized Home Health Intake Coordinator to join our team. This position is responsible for scheduling patient visits, communicating with doctors, hospitals, and home health staff to ensure smooth coordination of care.

**Responsibilities:**

* Schedule patient visits and assign clinicians
* Communicate with physicians, hospital discharge planners, and staff
* Enter and update patient information in DSL and Synergy systems
* Ensure all required documents are received and complete
* Follow up on referrals and authorizations
* Maintain accurate records and timelines

Job Requirements:

Minimum 2 years of experience in home health
* Fluent in English (spoken and written)
* Knowledge of medical terminology
* Experience with DSL and Synergy systems
* Strong communication and organizational skills
