Home Health Intake Coordinator Needed - Sherman Oaks, CA

We are seeking a self-motivated, experienced Intake / Front Office Coordinator to lead our front office operations, including intake, scheduling, posting, and care coordination. This is a key role that supports both clinical and administrative teams to ensure smooth and compliant operations.

Key Responsibilities:

Manage all patient referrals and intake coordination
Follow up on referrals from hospitals, facilities, and physicians
Schedule field clinicians and maintain up-to-date visit calendars
Handle posting and reconciliation of visits in EMR systems
Track and follow up on clinician notes and documentation to ensure timely submission
Communicate with physicians, hospitals, case managers, patients, and staff
Coordinate care for both Medicare and hospital/insurance-based patients
Ensure regulatory compliance and maintain accurate records
