Job Description:

We are seeking an experienced Home Health Office Coordinator / Intake Coordinator to join our team.
The ideal candidate must have prior Home Health experience and be familiar with Home Health documentation and daily office operations.
Responsibilities:
Assist with patient intake and referral processing alongside the existing Intake Coordinator.
Complete and review patient charts to ensure accuracy and completeness.
Process physician orders and maintain medical records.
Coordinate with physicians, hospitals, nurses, case managers, and other healthcare providers.
Perform daily Home Health administrative tasks and provide support to the clinical team.
Ensure documentation is completed in a timely manner and complies with agency policies.
Assist with additional office duties as needed.

Job Requirements:

Previous Home Health experience.
Strong organizational and multitasking skills.
Excellent communication and computer skills.
Ability to work independently and as part of a team.
Experience with Home Health EMR software Synergy is preferred
