HHA Intake Coordinator Needed Across the Street from GCC - Glendale, CA

Job Description:

Intake Coordinator of Home Healthcare Agency located in Glendale, across the street from Glendale Community College (students welcome to apply). Workdays: Monday – Friday; Work hours: 10 am - 4 pm.

DUTIES -
1. Intake of patients, check Medicare eligibility.
2. Prepare admission package, request medications from pharmacies.
3. Communicate with physicians' offices, drop off and pick up documents.
4. Coordinate, Schedule, and Post clinician's visits (SN, PT, OT, CHHA, SMW).
5. Monitor timely note submissions.
6. Export OASIS & Claims.
7. Maintain clinicians' HR file, update credentials, follow up on skill evaluations.

NOTE: Intake coordinator will be the only employee present in the room, and must be able to work independently, effectively communicating with the case managers and patients in English and Armenian. We do not provide training.

Job Requirements:

- No formal education level or license is required.
- Fluently operate either Healthcare Synergy/Kinzer EMR, including scheduling tools, maintaining clinicians’ libraries, etc.
- Understand home healthcare service process and flow-chart in details.
- Operating Microsoft Word, Excel, as well as Adobe Acrobat Pro.
- Fluency in English and Armenian.
- Effective communication and organizational skills.
- Self-reliance, strong sense of responsibility, and ability to work independently.
- Work ethics.

Salary:

$16.5 - $20 per hour
