As the Asthma Home Health Program Coordinator, you will play a crucial role in improving the lives of asthma patients by providing specialized care and education within their homes. You will be a champion for our patients, ensuring they receive the highest standard of care and support to manage their asthma effectively. This role combines clinical expertise with program coordination, allowing you to directly impact patient outcomes and drive program growth.
Key Responsibilities:
• Program Development & Coordination:
o Develop and implement a specialized asthma home health program, aligning with Soteria’s mission and CalAIM initiatives.
o Drive innovation in asthma care delivery and service models to enhance patient outcomes and satisfaction.
o Develop and implement strategies to expand the Asthma Home Health Program and reach underserved populations.
o Work to engage local primary care practices to better serve shared patients.
• Patient Assessments and Care Planning:
o Conduct thorough initial assessments and ongoing evaluations, developing personalized asthma care plans that align with each patient’s health goals, family dynamics, and environmental conditions.
o Focus on environmental asthma triggers within the home, and educate on how to mitigate them.
• Asthma Education and Management:
o Educate patients and their families on asthma triggers, medication adherence, proper inhaler technique, and environmental control measures.
o Provide education on peak flow meter usage, and asthma action plans.
o Provide remote education and follow up via telehealth.
• Medication Management and Administration:
o Administer asthma medications accurately and safely, educate patients on medication adherence, and monitor for potential side effects or adverse reactions.
• Patient and Family Education:
o Empower patients and their families to be active participants in their asthma management.
o Provide resources to improve home air quality.
• Interdisciplinary Collaboration:
o Coordinate care with physicians, respiratory therapists, social workers, and other healthcare professionals to ensure comprehensive care and support.
o Act as a strong patient advocate, promoting a whole-person approach to health.
• Documentation and Compliance:
o Maintain accurate and timely documentation of patient assessments, interventions, and outcomes according to agency policies and regulatory requirements.
o Ensure compliance with Medicare, Medi-Cal, and agency standards.
• Quality Improvement and Professional Development:
o Participate in performance improvement activities, case reviews, and training to stay updated with best practices in asthma care and home health nursing.
o Support program growth and contribute ideas to enhance patient outcomes and care delivery.
• Case Management and Resource Coordination:
o Assess and coordinate patient resources, from medical equipment to social support services, to support patients' comprehensive needs.
o Connect patients with resources to improve home environments.
• Patient engagement:
o Engage patients in the home, and remotely via telehealth.
