Job Description:

We are currently seeking a Full-Time Licensed Vocational Nurse (LVN) to work in our Home Health and Hospice offices. This position involves field-based work, where you will provide direct patient care in various home settings. While prior experience in home health or hospice care is preferred, we are willing to provide training for the right candidate.
We require to provide nursing care to patients in their homes, following individualized care plans as well as assist with patient assessments.

Job Requirements:

-Active and unrestricted Licensed Vocational Nurse (LVN) license
-Experience in home health or hospice care preferred, but not required
-Excellent communication skills and ability to work with diverse patients and families
-Strong organizational and time-management abilities
-Valid driver’s license and reliable transportation (car provided if necessary)
