-Verify insurance eligibility and benefits (Medicare, Medicaid, private insurance, HMOs).

-Ensure all required documentation is complete and meets regulatory and agency standards.

-Coordinate with physicians to obtain and confirm orders, face-to-face documentation, and certifications.

-Initiate the creation of individualized patient care plans in collaboration with the clinical team.

-Assign appropriate clinicians based on patient needs, location, and staff availability.

-Enter and maintain accurate patient data in the agency’s Electronic Medical Records (EMR) system.

-Communicate regularly with referral sources, patients, families, and internal departments to facilitate smooth transitions.

-Monitor the intake pipeline and ensure timely admissions.

-Ensure compliance with CMS regulations and accreditation standards (e.g., CHAP, Joint Commission).

-Follow up on pending documents and referrals to avoid delays in patient admission.
