Job Description:

Are you a detail-oriented and compassionate professional with a background in home health care? We are seeking an experienced Intake Coordinator to join our growing team and be the first point of contact for patients entering our care.

Key Responsibilities:
- Manage incoming referrals and initiate the intake process
- Communicate with physicians, hospitals, and case managers
- Coordinate with clinical staff to ensure timely admissions
- Filling/Charting all patient documentation

Job Requirements:

Minimum Qualifications:
- High school diploma or GED required
- Must know fluent English, no exceptions
- Minimum 1 year of Home Health Care/Office experience is a must, no exception
- Strong knowledge of anatomy, physiology, and medical terminology
- Proficiency in using electronic medical record systems
- Excellent verbal, written, communication and computer skills
- Experience with DSL is a plus!
